3.3.5 Front matter.
3.3.5.1 Front cover and title block page. The front cover and title block page shall be prepared in
accordance with figures 1 and 2. When specified by the contracting activity (see 6.2), an illustration of the
equipment shall be included on the cover.
3.3.5.1.1 Distribution statement, destruction and export warning notices. DMs shall have a distribution
statement and, if applicable, destruction and export warning notices on the front cover and title block page as
specified by the contracting activity and in accordance with MIL-STD-38784 (see 6.2).
3.3.5.1.2 Supersession notice. When the depot manual supersedes other manuals or portions of manuals,
a supersession notice, as furnished by the contracting activity (see 6.2), shall be placed as follows:
a. The statement "SUPERSEDES COPY DATED (insert date)" shall be placed on the DM cover
immediately below the DM number. (See figure 1.)
b. The complete supersession notice, in accordance with MIL-STD-38784, shall be placed on the title
block page. (See figure 2.)
3.3.5.1.3 Reporting errors and recommending improvements. A "reporting errors and recommending
improvements" statement (boxed-in) shall appear below the title data on the title block page as shown on
figure 2.
a. Unclassified DMs. For Unclassified DMs, the following statement shall be used:
"REPORTING ERRORS AND RECOMMENDING IMPROVEMENTS
You can help improve this manual. If you find any mistakes or if you know of a way to improve
the procedures, please let us know. Mail your letter or DA Form 2028 (Recommended Changes
to Publications and Blank Forms), located in the back of this manual directly to: (insert name
/ address of proponent). You may also send in your recommended changes via electronic mail
or by fax. Our fax number is (insert DSN and commercial number of proponent). Our e-mail
address is (insert e-mail address of proponent). A reply will be furnished to you."
b. Classified DMs. For classified DMs, the following statement shall be used:
"REPORTING ERRORS AND RECOMMENDING IMPROVEMENTS
You can help improve this manual. If you find any mistakes or if you know of a way to improve
the procedures, write or tell us about it. Address your correspondence to (insert name and
address of proponent). When dealing with classified information, make sure that your
correspondence is properly marked and is handled in accordance with current security
regulations."
